Department of Veterans Affairs

In 2007, an IT specialist lost a hard drive from the Department of Veterans Affairs that contained over 1.5 million unencrypted records of patients and healthcare providers. Identity Force was brought in to help craft notifications letters to this large group, set-up call center support and manage the identity protection enrollment and fulfillment process. Identity Force provided the VA with monthly management reports detailing the program’s usage and remained an active partner and advocate.

Montifiore Medical Center

Montifiore Medical Center wanted to make sure it’s systems and policies complied with the recently enacted Red Flag Rules. Identity Force worked with the Montifiore team and provided a comprehensive risk assessment and gap analysis of current patient identification procedures. The results of these studies shaped Montifiore’s new policies and procedures. Identity Force then created training for over 1,800 employees to support the new program.

Transportation Security Administration

The TSA’s Office of Human Capital lost an external hard drive potentially exposing the social security numbers, direct deposit bank information and payroll data of 100,000 employees — including airport security officers and federal air marshals. Identity Force was chosen to provide those effected with credit monitoring services, credit reports, fraud alerts and access to fraud specialists and identity restoration specialists. At the conclusion of the one year contract, the TSA opted to extend the benefits for a second year, in part due to Identity Force’s 100% success rate in restoring the identities of all affected employees.

National Institutes of Health

During a clinical trial at the National Institutes of Health, a laptop containing seven years of data was stolen from the trunk of a car exposing the personally identifiable information of 2,500 people enrolled in the study. Identity Force mailed letters on behalf of NIH to notify patients of their customized identity theft protection program, which included three bureau credit reports, $25,000 of identity theft insurance for each patient and complete identity restoration assistance.

Iona College

Recognizing the increasing threats of data reaches and identity theft at colleges and universities, Iona College proactively set out to become the first “data breach free campus” in the country. Iona’s program encompasses training for staff to reduce the threat of data breaches, compliance with all federal and state laws, financial literacy education, reputation management, and identity theft protection, for students, faculty and staff.

Wuesthoff Medical Center

Names, social security numbers and personal medical records of over 700 patients at the Wuesthoff Medical Center were posted on a public internet site. Believing their pre-registration patient Website was hacked, Wuesthoff Medical Center hired Identity Force to provide credit reports and daily credit monitoring, fraud alerts, access to fraud specialists and restoration advocates, as well as $25,000 in identity theft insurance per victim.

Walter Reed Army Medical Center

Unauthorized peer-to-peer file sharing inadvertently exposed names, social security numbers and other personally identifiable information of nearly 1,000 patients at Walter Reed and other military hospitals. Identity Force was called in to provide emergency breach response services including a full suite of proactive credit and identity monitoring, customer service and identity theft insurance to everyone who was affected by the breach.
